PRIVATE HEALTH INSURANCE 

Resolve Chiropractic currently accepts most private insurance company plans and is always working to become an in network provider with other major insurance companies in the SE Wisconsin area. Please contact your insurance provider to confirm Resolve Chiropractic is in your plan's network. Out of network paperwork can also be provided for patient reimbursement through their health insurance provider. 

MEDICARE/MEDICARE SUPPLEMENT 

Resolve Chiropractic accepts Medicare insurance including Medicare Advantage plans, Medicare Complete plans, and Medicare Supplement plans. 

WI STATE INSURANCE/FORWARD HEALTH 

Resolve Chiropractic accepts Forward Health, Bagercare+, and other Medicaid plans 

PERSONAL INJURY/WORKERS COMPENSATION 

Resolve Chiropractic with work with your insurance company to ensure that you receive the care you deserve to get back on your feet and back to living your life after an accident or injury. 

NON-INSURANCE OPTIONS/HSA

Resolve Chiropractic accepts forms of payment for services not being billed through insurance. All major credit cards are accepted as well as Health Savings Account (HSA) payments. 

HARDSHIP RATES 

Resolve Chiropractic believes that everyone should be able to receive chiropractic care regardless of their current financial situation. If you feel that chiropractic care is not attainable due to financial stressors please contact the office to see if you are eligible for discounted hardship rates.
